Mouse infestation was present. Feces in basement storage room observed on floors and shelving.
No hot water was provided at sinks throughout establishment.
Employee hand wash frequency was inadequate, due to lack of hot running water.
Multi-use utensils were not properly cleaned and sanitized, due to lack of hot running water.
